Registration

To apply to be part of the T.E.A.M. 21 program, a parent/guardian must complete and return a two-sided application.   Applications are available at school or on this website.  All completed applications should be returned to the school’s T.E.A.M. 21 site coordinator.  Students accepted into the program will be notified by via phone or mail.  If capacity in a T.E.A.M. 21 program is reached, a waiting list is maintained and students will be enrolled as spots open.
